FORMS
The Ontario government (OHIP) created packages for patients with prosthetic eyes, conformers, adjustments, build-ups, and recolours to aid our community in having the best health care provided by Ocularists. This includes government funding, (ADP) providing 75% of the total cost of a new prosthetic eye, while the patient pays 25% of that total cost. You may submit the portion of 25% to your private insurance company with the invoice you will receive upon the appointment day of payment.
This cost will vary as every patient is unique, with unique surgeries.
We offer a free consultation to further discuss your unique case, as well as an estimated cost before beginning the appointments of creating the prosthesis.
If you are traveling from afar, outside of Toronto, please download the travel grant below (supported by the government). Please have your family doctor sign the form in order to receive the grant.
The Assistive Devices Program (ADP) form must be completed in order to receive the 75% coverage. As well as, have your doctor sign before arriving to your appointment for a new prosthetic eye.
