Your First Consultation

We encourage all new patients to first book a consultation. During this consultation, you will meet with your Board Certified Ocularist to determine if you are a good candidate for custom ocular prosthesis/scleral lens prosthesis.

They will typically go over the process, device costs, as well as answer any questions you may have to ensure you are making an informed decision before proceeding. 

You will then see our Office Manager who will give you the necessary paperwork, cost estimates, and book a series of fitting appointments.

First Consultation Checklist:

  • Health Card

  • ODSP/OW statement (if applicable)

  • WSIB Claim number (if applicable)

  • NIHB information (if applicable)

First Appointment Checklist:

  • Completed forms (original, not faxed or photocopies): Assistive Devices Program form and/or Alcaine Approval form
